Thyroid hormones are synthesized in the thyroid gland with a process of iodination and protein binding. Protein binding is with thyroglobulin which is synthesized and stored previously in the thyroid gland. When the hormone synthesis occurs, throglobulin is delivered from the follicular lumen where it is stored to form thyroid hormone precursors, monoiodathyrosine and diiodothyrosine (MIT,DIT) to become T3 and T4 with an oxidative coupling.
Interpretation:Serum Tg is increased in many patients with thyroid disorders like goiter, benign thyroid nodules, thyroid adenomas, multinodular goiters, thyrotoxicosis and in toxic phase of thyroiditis. It is also used to monitor patients with papillary and follicular carcinoma and following thyroidectomy.
