Ionised Calcium is the portion of calcium which is free (not bound to proteins). Calcium is a very essential ion for cellular transports, nerve conduction, many physiologic and metabolic reactions, for bone integrity, neuromuscular system, muscle contraction and relaxation, hormonal secretion etc. Ionised calcium is the protein unbound portion able to perform all the above functions.
Interpretation:High Calcium (hypercalcemia) is usually seen because of an overreactive parathyroid gland which is the regulator of calcium absorption, release of calcium from bones to the bloodstream and conservation of calcium by the kidneys. Measurement of ionised calcium may also help in the diagnosis of hypocalcemia where calciom depletion may impair electricity conduction in the blood and the nervous system in addition to all Calcium mediated function impairment of the body.
